Hospice of St. Lawrence Valley is seeking highly qualified candidates for the following positions. The team approach to care provided by Hospice allows employees to work collaboratively and to provide support to one another as needed. Please call us at 315-265-3105 if you have questions. Office hours are Monday through Friday, 8:00 a.m. – 4:30 p.m.

Bereavement Coordinator – Immediate Opening

Responsibilities include, but are not limited to:

    • Provide bereavement follow-up support to hospice families for thirteen months following the death of the hospice patient
    • Maintain appropriate documentation/records as required by Hospice and regulatory agencies
    • Engage in activities related to the development and coordination of bereavement services provided by Hospice of St. Lawrence Valley
    • Provide short-term counseling, and resource and referral services to bereaved families/primary care givers
    • Implement and facilitate bereavement support groups and coordinate bereavement functions to include support for health care workers, first responders, and workplaces, as needed
    • Develop and produce community grief education programs and material
    • Work with the Volunteer Manager to develop and implement a grief volunteer program
    • Participate in agency and community education and outreach programs to promote professional growth and community understanding and support of hospice bereavement care
    • Respond to schools in St. Lawrence County when a student or staff member has died
    • Provide additional resources and education to area schools as needed
    • Develop educational programs, materials and library resources for staff, volunteers, and families/primary care givers
    • Develop appropriate hospice events in cooperation with other departments and agency staff
    • Participate in the orientation, training and coordination of hospice staff and volunteers related to grief education or the provision of bereavement services
    • Serve as agency representative on local, regional and state committees and inter-agency groups as assigned
    • Attend all appropriate in-house meetings and participate in relevant community meetings
    • Accept other responsibilities as deemed necessary and appropriate by the Director of Family Support Services
    • Supervise bereavement support staff and volunteers
    • Participate in staff training and educational programs related to the Compliance Plan and adhere to the organization’s Standards of Conduct

Registered Nurses – Full Time and Per Diem positions available

Responsibilities include, but are not limited to:

  • Providing nursing for hospice patients
  • Educating family caregivers and providing support as needed
  • Developing plans of care to manage symptoms and maximize quality of life for patients
  • Continuous assessment of the changing needs of the patient and family and providing services according to those needs
  • Accurate documentation of assessments, changes in plan of care and results of interventions to address the needs of the patient which are regularly communicated to the interdisciplinary team
  • Supervision of LPNs who may be providing skilled nursing
  • Participating in training and continuing education to maintain the highest standard of care for patients
  • Acting as a representative of Hospice of St. Lawrence Valley in the community

Qualifications:

  • Current license to practice as a Registered Nurse in New York State
  • 2 years experience in acute care setting
  • Valid New York State driver’s license

Social Worker – Part Time, Immediate opening

Responsibilities include, but are not limited to:

  • Provide end of life counseling and support directly to hospice patients and their families
  • Develop plans of care to meet the psycho-social needs of the patient and their family
  • Assist team members to understand the significant psycho-social and emotional factors impacting the terminally ill patient and the family/caregiver. – Collaborate with team members regarding issues related to anticipatory grief and bereavement counseling
  • Assess patient/family/caregiver social and emotional factors to determine their capacity and potential to cope with the terminal status and death of the patient
  • Attend and participate in IDG meetings to coordinate with team members to develop and implement an appropriate plan of care and to update absent team members
  • Provide counseling services and crisis intervention as needed to the patient/family/caregiver
  • Assist patient/family/caregiver in locating and accessing community resources as necessary and appropriate, as well as advocate for their needs
  • Educates patient/family/caregiver about psycho-social issues
  • Provide discharge planning services

Qualifications:

  • BSW, LMHC or LMSW
  • Relevant  professional experience in interdisciplinary settings
  • Valid New York State driver’s license
