Advance directive, living will, health care proxy, power of attorney…what do they all mean?
In recognition of National Health Care Decisions Day, Hospice will offer an opportunity for community members to come learn about what all of these terms mean and why we should all consider making some of these decisions.
Forms recognized by New York state will be available with assistance from staff.
We will even offer some tips on how you can have an important conversation on making your wishes known with your loved ones.
Community Members are welcome to stop in at the Hospice Center on Thursday, April 14th from 1-3pm to learn more about making health care decisions.
For more information contact Nancy Vosbrink at 265-3105.
