Hospice of St. Lawrence Valley, Inc. is seeking a part-time or per diem Pastoral Care Coordinator to coordinate, plan and provide spiritual support and counseling services to patients, families, and caregivers in keeping with their belief systems. The Hospice Pastoral Care Coordinator acts as a liaison between local clergy and the interdisciplinary team. Hospice must adhere to Medicare conditions of participation. The Hospice Pastoral Care Coordinator will assess and identify the patient/family/caregiver spiritual status and needs; provide spiritual care services to patients/families/caregivers; consults and collaborate with members of the interdisciplinary team in the development and updating of the patient centered plan of care; participate in interdisciplinary team meetings; assure that all documentation meets requirements for accuracy and timeliness, and reflects services provided, patient/caregiver needs, and confidentiality; contact area clergy when clergy are requested by patients or families; conduct and/or plan memorial services for patients when indicated; and conduct individual Veteran Pinning in collaboration with the We Honor Vets program.
Qualifications include BA or graduate of accredited seminary or school of theology or lay ministers, deacons, or related field; three years of local church pastoral experience or its equivalent; ability to work with all faiths and denominations; and demonstrated ability to be open, sensitive, flexible, and ecumenical. Being technologically inclined is a must, plus willingness to learn how to use Hospice’s electronic medical records system.
